Cash or Debit /Credit cards / BACS transfer -WE DO NOT accept American Express or  cheques

Please bring the following with you:

• A completed form 

• A patient summary from your GP

• A valid passport for ID purposes

• Last Medical Certificate- if applicable

• Glasses/ contact lenses if worn with an up to date spectacle prescription 

• A Chaperone if you wish

• A means to pay for the assessment on the day
